Screening From The Convenience Of Your Home
 

The home test kit is an employee-administered health screening that does not include health coaching as part of the base program*. A toll-free number is provided for employees to call for more information and support. Click here to see details of the Home Test Kit Program.

As an option, clients can arrange to have Summit Health provide telephonic-based coaching for employees who participate in home test kit health screenings. Coaching programs include:

  1. Standard coaching similar to that provided at the on-site Healthy Heart Program, which takes approximately five minutes
  2. Extended coaching that can include review of Health Risk Assessment (HRA) results and generally takes 15 to 30 minutes

This coaching occurs after employees receive their home test kit results, which are provided both electronically and by mail. Summit Health coaches contact employees by phone to discuss their results and appropriate next steps. Telephonic coaching for home test kit screenings is similar to coaching at on-site screenings. During telephonic coaching sessions, coaches discuss participants’ screening results to ensure that participants understand results that indicate they may have elevated health risks, and are provided with information about disease prevention and wellness maintenance.

Customized coaching options also exist for the home test kit.

* Please note that the Home Test Kit standard program provides for Summit Health staff to contact employees whose biometric screening results exceed recommended thresholds. Summit Health medical staff contacts employees by telephone to notify them abnormal results and recommend they contact their Primary Care Physician (PCP) as soon as possible.
