Summit Health custom developed a home test screening kit for the following:

1. Lipid Profile (Total Cholesterol, HDL, LDL, Triglycerides) and Glucose
2. Lipid Profile (Total Cholesterol, HDL, LDL, Triglycerides) and Hemoglobin A1c
3. Prostate Specific Antigen (PSA)

The employee administers the test in the convenience and privacy of his or her home. The Home Test Kit is mailed to the employee’s home or office, where the employee self-administers the test by placing blood on a specially treated paper the size of a credit card. The employee then mails the specimen in a self addressed and pre-paid envelope directly to our lab for processing and reporting. The entire process is safe and easy, and the employee may call an 800 number at any time for more information and support. After the blood sample is processed, our lab mails the test results and interpretation to the employee. The laboratory then sends Summit Health the results electronically. Summit Health uses this electronic report as part of the client’s aggregate report and stores and retains the individual’s results for telephonic follow-up with the participant.

Everything is conveniently included in the Home Test Kit, including forms and instructions, finger stick supplies, and a preaddressed, prepaid mailing envelope for the blood sample.
